Professional Disclosure Statement for Clinical Supervision

Before beginning a supervision relationship, I provide all supervisees with a Professional Disclosure Statement outlining my qualifications, approach to supervision, expectations, responsibilities, confidentiality, and supervision process. I believe transparency creates a strong foundation for an ethical, collaborative, and trusting supervisory relationship.

This document is reviewed together before supervision begins, and supervisees are encouraged to ask questions at any point throughout our work together. If you have any further questions or thoughts please feel free to book an exploratory call with me to determine if we are the right fit.

My Counselling Background
● 6 years postgraduate experience
● I work with youth and adults in individual counselling. Eating disorders, trauma, complex trauma
and PTSD, attachment wounds and human trafficking are my areas of speciality
● I operate from a psychodynamic theoretical orientation and use a mix of modalities including EMDR, OEI and DBT therapy along with somatic based interventions.

Nature of Supervision

Clinical supervision is a dynamic, relational partnership designed to promote your professional growth while ensuring the highest standard of client care. I view the supervisory relationship as inherently collaborative, educational, and deeply supportive. We both carry distinct, active responsibilities throughout our work together. Because supervision naturally carries an objective power dynamic, I actively work to name and
deconstruct it. My goal is to create a secure, non-defensive space where you feel safe enough to share your mistakes, uncertainties, and ethical edges without fear of judgment. Within this space, my role balances four key pillars:
Support: Holding space for the emotional weight of clinical work.
Teaching: Integrating theory, skill acquisition, and ethical decision-making.
Evaluation: Providing transparent feedback on your clinical competency.
Gatekeeping: Stepping in with clear, structured guidance if areas of improvement or ethical concerns arise to protect both the public and your professional standing

Our work is grounded in a trauma-informed, relational, and developmental model of supervision. This means I meet you where you are in your career stage and that supervision will fluctuate and grow with your needs over time. We will collaboratively develop our own supervision relationship, and your feedback is always welcome.

Client confidentiality will be maintained as per the Standards of Practice and Code of Ethics for Counselling Professions. The issues you discuss in supervision will be confidential with the following exceptions:

1) Where I am required to provide formal evaluations regarding your performance and conduct in this clinical supervision experience or when consultation with another professional is necessary.
2) When information is required for professional association or regulatory body registration.
3) If I am asked to provide information about your clinical experience in the form of a
recommendation/reference for a job, licensure, or certification.
4) Disclosures made in triadic or group supervision cannot be absolutely guaranteed as
confidential. Although I will take every measure to encourage confidentiality and act appropriately if confidentiality is not upheld.

Anonymity of the supervisory relationship is limited, and the following exceptions may occur:

  • Joint clinical research, authorship and project participation.
  • Interagency and possible community social events.
  • Attendance of professional development workshops, seminars, conferences, etc
  • Membership in professional associations/regulatory bodies; adherence to
    Code of Ethics/Standards of Practice
  • Liability coverage of supervisor and supervisee.
  • Inform clients that the supervisee is receiving clinical supervision.

Clarify gatekeeping responsibilities as a professional duty.

Your commitments to Supervision: Supervisee’s Responsibilities in Supervision:

  • Active Engagement: Arrive prepared for our sessions with specific case consultations, clinical questions, or themes you want to explore.
  • Reflective Practice: Deepen your self-awareness by engaging in self-reflective exercises, intentional homework, and ongoing personal and professional development.
  • Clinical Transparency: Keep me informed on client progress, case conceptualizations, and evolving clinical issues.
  • Proactive Risk Management: Promptly report and consult on any high-risk client situations or complex ethical dilemmas.
  • Skill Cultivation: Record counselling sessions (with appropriate client consent) and complete thorough tape critiques to objectively review your clinical interventions.
  • Professional Liability: Maintain active professional liability insurance at all times and collaboratively complete the supervision record during each of our meetings.

My Commitments to you: Supervisor’s Responsibilities in Supervision

  • Structured Mentorship: Prepare for and provide a clear, supportive structure for each supervision session, ensuring your goals, clinical growth, and client care remain the priority.
  • Constructive Feedback: Provide real-time, actionable feedback during our sessions, alongside formal evaluations as required by your regulatory body or at the conclusion of our contract.
  • Evaluation & Review: Review recorded sessions, case conceptualizations, and documentation to support your clinical growth and protect client welfare. Provide insight and support for areas of growth.
  • Person-of-the-Therapist Development: Dedicate space to exploring your internal landscape as a practitioner—helping you navigate countertransference, prevent burnout, and build a sustainable practice.
  • Professional Standing: Maintain all necessary credentials, liability insurance, advanced training, and active supervisor standards required to formally support your registration journey.
  • Documentation Compliance: Jointly complete and sign off on the official supervision record at the end of every session so your hours are accurately tracked.

Session Fees and Length of Service 

Standard individual supervision sessions are 50 minutes, however longer durations can be requested. A 50 minute session is $175 and the fee for the session can be e-transferred at the end of the session time. You will be provided with a receipt within 1 business day of receiving the payment.

I believe that the supervision relationship flourishes over time as we learn to work together and I learn how to best support you. My recommendation is commitment to once a month supervision sessions, however there are times where you may need different frequency of sessions and so we will decide together our supervision frequency to meet your needs.

Emergency Contact

You are welcome to reach out to me in an emergency situation by email, though there is no guarantee I will be able to answer outside of my business hours (Monday-Friday 9-4pm). I encourage you to have other colleagues to reach out to in the case I cannot provide supervision as soon as you need it.

Complaints
▪ Supervisees are encouraged to discuss any concerns with me first and engage in a collaborative resolution process.
▪ If this process is unsatisfactory, you may contact my immediate supervisor or file a complaint with the professional association or regulatory body to which I belong
